Because he gave something wrong to the mc. The bacon (or what it was) made the mc feel bad, the man didn t know and so he told the doctor he wanted "to reflect on himself" (he felt sorry, he didn want mc to feel bad). He is kneeling to show how sorry he is, he is doing "reflection" on himself. Obv the mc Is way too scared of the man to care about that, the doctor knows that the act of reflection is not useful and that he should just go away and leave the room. (Only this way the sick mc can relax)
