The current FL is not the wife in terms of their body. She ONLY HAVE THE WIFE'S MEMORIES. Losing her virginity a second time and Duke mentioning FM being younger that how he remembers is proof that FL is the younger version of the wife = This FL didn't married and gave birth. The protagonist is the regressed version of the wife. Arguably the wife disappear and only her memories is left.
They explain this more in the novel that when she gave birth and had a chance to go back, both body, mind and time regressed to how she was before she got isekaid.
The wife after returning to her world and regressed back in time and body. The Duke's father asked him to tell everyone the wife died. I haven't read the last chapter of the novel yet but the latest one I read, it was implied that the father of the Duke did not approve of the wife because she's human and and used her only to bear them an heir. Who knows maybe she was also threatened by the father of the Duke.
Thus they implied that the behind the Duke's back and in the shadows, the father prob told the wife she was only wanted for an heir and not believe the Duke's affection ergo it is much easier for her to let go of them.
As for why she left them. The current female lead foreshadow it already: she grew up conditioned to be a responsible person. Plus her last parent is dying and have only her for support.
Take these into account and we can tell she chose to go back and leave because 1) she wasn't fully welcomed by the Duke's family 2) for her it is irresponsible to leave the way she did 3) her responsibility was first before her responsibility with to husband/kid for timeline reason.
Even now it's the same... FL wants to go back first of all to take care of her parents. Reason why her deal consent to being a sex slave for the Duke literally guarantee only her return.

As for mine, well I apologize if some readers got caught off guard. For me, everything already points to her being the actual wife. Flashbacks, the maid and butler treating her kindly, her being a spitting image of the wife, almost identical names, moles, body, mannerisms, her feelings toward him and the child, them being obsessed with her, her worrying about her bedridden mom, everything. It’s obvious.
But never did I think her body regressed. I thought maybe amnesia or her OG world and his have different timelines (like one moves faster and one slower)
