This is true. All for one bestowed a quirk fir physical enhancement/storage of physical power on his seemingly quirkless brother. But the brother already had a quirk for transfering quirks so both quirks mingled in his body and One for all came into existence. This was told in one of the chapters.

The doctor sure is working with all for one , either by transforming children into nomos ( like the winged nomo which was clearly identified as Izuku's childhood friend ) or maybe be simply just taking their quirk (I doubt that option because no one other than Izuku is showen to be quirkless )
There is a panel where All for one telling all might that he took something important from all for one :/ what did all might take to make afo this furious ? Something related to Izuku maybe ?

About that first question, All might went and wiped off all those people who were following/related to All for one. That was when All might was trying to find All for one. You know the fight where All might got poisoned. That was a how it all went down. As far as I know All might went after All for one bc he killed Nimura or something. So its kinda revenge going back and forth. N I guess that was how Tomura got involved too. Since All might took everything away from All for one, now he is trying to hurt All might as badly as he can!
