East Asian BL is strict about top/bottom roles, so it does matter to most artists. Also the only reason why it's not r18 is because the Chinese gov bans r18 content, who knows they might imply they had sex in the manhua.
Though in the end I did some digging myself and I believe the psychologist is the bottom from some of the cover art/manhua panels but of course I'm not 100% sure since I haven't read it.
