i don't think the doctor is being manipulative because the brother and doctor meet 2-3 times only, and went only for drinking twice with each other, even then they don't talk to each other on a personal level. So basically the doctor or brother have no clue about each other's interests. For meeting someone for the first time, the doctor actually treats the brother pretty well. Comparing to a lot of semes have I seen, the doc is actually a good seme, he takes care of a drunk uke and sleep with him consensually and tries to understand the brother and help him out as mental support.
