i think that arc would be pretty large because i think that the reason why the doctor stop being a doctor was because he make a mistake operating natsuo's mom so its going to get dramatic

in that case i wouldn't mind to have '' large arc '', because the main couple a little boring ( don't get me wrong ) I love them and they are cute but from 1 ch. endtil now nothing interesting happened in their story, so I really prefer to have a long arc about Natsuo& Sensei, since they are my favorite.

and if the reason like you said, so I can't wish a better story to see how they become a lover
