Yeah I think the last few panels were meant to show that Todo had no reason to be insecure (the girls like him and his friends are understanding of his situation) but the seme used his insecurities to make him feel like he was the only one that would accept him and even faked his condition to get closer to Todo
After rereading this... Could it be that last image of the razor blade mean that he actually had no problem, and the "not having public hair" disease was a lie to get close to Todo?
