I thought that was like her inner self, and that it was her heart she was holding, which meant she gave her heart to him (and put herself in prison).
John have Annete the heart of her husband and of the old man, so I thought it must be a reference of that
Ok but I'm confused with the ending scene... who was the one seeing them from behind the tree and what's with the hole in the stomach... is it indicating a possible birth of a child or death???
