Reminds me of that BL "Ordinary Men". It doesnt have the "oh shit, am i gay?" and real world homophobia. Just people having relationships. And the fact that their relationship is normal means the setting doesn't really demands an heir (in his father's case, it was his obsession with weakening the curse), with all these patrons and possibly adoption, there's also the younger brother as potential next heir coz he's young.
The story and art is great and all, but if you change MC into a lady, it wouldn't be any different. Especially when they're established fiancés when there's not really much benefit since they can't have children, the situation is just odd for the both of them.
