It’s still a big thing. Irl having children or not can be a huge deal breaker for a relationship. If a couple were dating and hoping for kids, but the one of them is infertile, that could cause them to break up due to wanting a biological kid. The MC’s reasons for worry are valid in a way. As well as the fact that the ML didn’t actually approach the MC with romantic interest until after he became an omega. MC’s worries and insecurities are reasonable even if he’s been told by the ML that he loves him. It’s natural to have moments of insecurity in a relationship no matter how much reassurance you receive. It’s not a matter of distrust towards his partner but only an internal struggle with himself.
