Before she did not refuse. She lived for the emperor. Did everything for him. He had no reason to doubt her. This time she refused and and then changed her mind. Anyone would be suspicious of that. He is horrible and we hate him but the solution he came up with is the best there is. He doesn't need to trust her when he has her life in his hands. She knew that and that is why she drank the poison.
I'm sure our boys will find the permanent cure eventually, but she has to suffer for the emperor to lower his guard so they can achieve the plan.
