I hear irl ppl clean themselves out first (prepping) but in yaois I guess it would disrupt the flow and sexiness if they don't "get taken" right there. Although some comics does show them getting prepped. I'm assuming getting prepped isn't just getting your hole stretched out. Doesn't mean it's clean but it's better than nothing. By clean I mean it's not completely disinfected.
