The whole thing about Makoto's backstory is (From my own interpretation from translations and raws) Is that the man Makoto lives with now isn't his blood relative at all, and was just a random man he used to play with in the playground when he was younger.
A fire had happened in Makoto's house, which seemed to have probably killed his parents inside, and seeing the opportunity, the man took Makoto to live with him, it seems that ever since, the man groomed Makoto and did the things Makoto does to Mikoto now.

why the fuck makoto gets a fresh new start while mikoto gets killed by one of her one night stands cos she continues getting off from nearly dying. what the hell
