I don't think it's actually his brother

I think it's himself, like he's hallucinating because of his overwhelming feeling of guilt? Plus his anxiety. I don't know for now but I think that dude doesn't truly exist, or that he doesn't exist anymore (died with the parents maybe idk), or he's an alter ego or something
