It's different. By making the choice to not "save her" it's like he is the one killing her. If he choose to save her, ML will just loose one leg, his dream and probably ruin his life. If I would take a wild guess, MC will try to sacrifice himself - again. If it isn't possible, I don't think he will be able to make a choice...
The sad part, it's that it will be random based on the last panel and probably the grandma as ML is too important to the story. :(
