Basically, the MC was born with the ability to conceive. It's a medical condition that also caused his weakened health state. His parents found out about it during his period of hospitalization, and decided to not tell him.
So, in reality, he's just capable of having kids? I'm gonna bet that condition is what the current medical technology for male conception is based off of. Like it's super rare and his local hospital isn't well-equipped to handle it.
In the end, he has to travel out of country (to the US) to find a hospital well versed enough to handle his care. So, definitely not ABO, but feels a lot like ABO without the pheromones/marking/mates bit.
