    Well STI's or STD's are transmitted when one or both partners in a couple have unprotected sex with multiple partners. So if the couple in this manga was in the real world then they would be safe for unprotected sex (assuming they are each others first). However protection is always the safest since the back door still has a lot of bacteria and nothing grantees your partners loyalty...

    STD and STI both essentially stand for the same thing. There may be a tiny difference between them as STI is more preferred in the health world, but many people use them interchangeably.

    Where you live also influences which one you potentially use because in the place where I live, we were taught using the term STD, hence we us it more frequently used over STI.
