ML's realization of MC's true identity was gradual. First because, even though there were countless "Abels" sent to him, they don't have informations about the things Abel and Karas talk about back at the Dragon's Forest. The things about "just rolling in the grass, smell the fresh air and just live in peace" things were exclusively said by the real Abel. Our MC also told to Karas that reincarnation is not real (he said this because Abel was too guilty of what happened to the past) despite the fake Abels telling Karas that they were reincarnated.
After the current chapter, Abel slept for days because of Fatigue, Karas tried to test his suspicions by calling him "Eppel" to wake him up. Abel woke up for a few seconds and responded to Karas with a smile, this was the time Karas was almost sure that Abel is real. The moment Karas became absolutely sure of his identity was when he tricked Abel in a shiproom they were together in that there's a wolf spider behind him and then Abel immediately passed out lmao, and at that moment, when Abel was unconscious due to the spider, Karas officially declared that "now you are mine".
As for Karas being cruel, like I said in a spoiler below, he's not your typical red flag ML. Karas is a warlord, and a savage one at that. He doesn't care if you are an innocent pregnant woman, an elderly, a child or even an INFANT, Karas won't hesitate to kill them. Karas also doesn't care if Abel hates him or love him, he also doesn't care if he dies with the black potion so long as he's able to monopolize Abel's attention all to himself by constantly thinking and worrying about him, basically he wanted Abel's affection and attention, even if they are miniscule, as long as Karas evokes some sort of emotion out of Abel, he doesn't really about anything. I pity Karas in a way, he's not the character that can be fixed, he's permanently damaged. For me, Karas is the ultimate red flag amongst the red flag MLs because his red flag-ness is not limited to his eternal obsession with Abel.
