As the others said, it always depends on the author's choices. Omegaverse can be vastly different in some stories compared to others
It really depends the idea the author has, tbh. An omega may be able to get pregnant, but it's not a life duty to do so and he just can do whatever he likes. Think of it as a trans man top x cis man bottom (not a 1:1 analogy, but you get the point)
