I think it's bcs she died still as the cheater wife or widow, so Rick and Nelly relationship an affair in the eye of nobility I think. But therefore Nelly doesn't want to be buried with the cheater. so that's way she and Rick make 2 grave, one is Nelly grave as the cheater wife that everyone knew(her body not there I assumed or maybe someone else body) and the other grave is the one that didn't have name(Nelly body in here).
She prolly does but since she was originally the wife of the brother they had to put her grave next to him for public appearances so they made a fake grave for that and her real grave is nameless so richard can stay with her
