I think there's some insane reason he had to leave (there are hints that the younger brother and best friend set up the "whole getting someone pregnant", but how could he even think that's true if he didn't fuck up in some way?), but he should've told her the truth. He's a selfish bastard and he intentionally left like that to make sure he will haunt her and he has a place to come back to. And to think he has been hurting her emotionally during the relationship.
