
In the story, if I can remember they did say that no one knows his name and plus it had been 10years since he dissapeared so prolly he came back very old, changed his name, his grade and his identity as an Esper and faked his identity as a Guide as well as a therapist.
I mean even for me it be hard to recognise a person who I've met 10years ago when they were younger with different name and different identity.

Then they should've designed him where it was harder for even readers to recognize him. If readers start to be suspicious of him just by seeing a short flashback mentioned that shows his long hair tied in a ponytail like that so easily, then it kinda kills the twist when it happens. They should've made the flashback model not easily recognizable like a dark figure or don't even see the hair or head at all and just the lab coat. It would've made the reveal a lot more surprising.

I wuld say, it was quite a dumb thing to make them look similar. I'm sumone who focuses more on the Art (I'm an artist) so it was quite easy for my to guess and in the end I was right.
Maybe if they had just shown the eyes under the therapist's glasses it be different. But the way the eyes was hidden made it more suspicion and even the conversation.
It feels more like a rush entrance of the intro of the therapist as the SSS esper.
Like they want to quickly introduce him as the lost SSS esper and be done with the sus situation
I knew that therapist was suspicious after I saw the darken drawings of the "first SSS ESPER" Doc. They were way too similar in hairstyle and outfit
