I'm not caught up on the comic because I'm waiting for chapters to build up so I don't know how far it is. Also, the novel's translation slowly got worse, so I'm unclear of the details at the end. Doesn't help that MC is mentally ill and often makes no sense.

It's nonstop drama
The main spoiler is that yes, he's in the novel his sister's friend wrote, but he's not the only one. His sister is his brother, and the author friend is still there. Also, one of MC's friends and the ML is his crush.
The sister/brother and the author have their memories
The friend had them in the beginning, but not anymore
The ML doesn't have them; he may have had them in the beginning and forgotten. - he does gain them at the end.
The reason the MC is in so much pain is because they all but him died in the mall collapse in the original world.
The MC is still alive, trapped, and slowly dying, and time just flows differently
He's still alive because ML/crush blocked his body from rubble, and is just on top of him, dead.
The story is a lot. At one point ML does SA MC, just in case that's something you avoid.
It's a 100% done out of anger and he regrets it immediately and it tears him up that the MC thinks he deserved it.
MC is an alcoholic and constantly puts himself in danger; the alcoholism and hiding it are what leads to ML's anger.
The ML just slowly looses his mind because he falls in love really quickly and the MC just keeps doing stuff that makes him sick or almost kills him.
By the end things are solved but ML has gotten to a point where he can't be separate from MC at all or sleep.
Things MC does
Alcoholism and hides it
Gets in the main character's business that gets him in trouble
Sees his family, even though it causes attacks- it would be fine if he were seeing them to get answers or because he misses them, but he goes home to pick up his HS uniform because he wants to wear it, and then has an attack in the middle of the street.
Gets kidnapped and loses memories of being in the book
Falls into the lake after being told not to wander off and ends up in a coma.
Never tells the ML where he is going and lies about what he's doing.
Obviously it's because the ML is overbearing but I pity him a bit.
He has a basement where he's trapped a doctor? Maybe more? And they just have to come up with a way to cure MC. I'm not sure about the details because MC is a very confusing storyteller.
Wow no wonder the title is gloomy delusion. The story is sorrowful. Thank you so much.
