Since the setting is historical fantasy, they probably see the birth mark as a demonic mark or a cure mark. To them it’s a sign bad luck and if they overly religious, then someone who was rejected by their God(s).
True, but they would have mentioned that in the first place. They just called him ugly.
WHAT THEY HELL IS WRONG WITH THESE PEOPLE?! IT'S A BIRTH MARK! Did they never think to put make up on him, or just cover up that one side with a mask. Calling him a monster when he hasn't done anything is what is grotesque! All those people too! But not him!
