Why do most of them want to hurt the doctor?

The concept of espers and guides are relatively new, and most of them were subjected to inhumane experiments to test them. Not many actually trust the medical side due to this, and the doctor who was in Mc position was kidnapped and tortured revealed secrets too. There is very little trust in doctors.
