I actually think the author is consistent with the character's experience. It's true that in four months he completely changed his appearance, but he still remains the shy and introverted boy he was before. I know people who have gone through similar journeys (although with weight loss over a longer period), and even after years, they sometimes still have irrational fears caused by the trauma they experienced when they were obese.
Men, Pink Hair is such a coward it's pissing me ooooff. If you want it, then take it! Especially if it's being handed to you in a silver platter! And if you can't find the courage to take it, then give up man. Stop acting like the victim when you're the one who's blocking your way to happiness.
