I meaaaaaaaan technically I can rec "rainbow city" because it deals with cults, corrupt government, dna experimentation and torture, fake vaccines and mutating viruses
ML had to kill his entire family due to them being infect, watch a guy who helped him get shot right in front of him, and lived his entire childhood in hiding because he was born without the cities permission, and when he did get found by the city, he was used as a human meat shield watching all his comrads die and slept in maggot infested cages as punishments, all in exchange for filling his stomach, and he's also faking playfulness throughout the entire story

MC grew up in better circumstances but still had a birth defect of having a weak body that could die just by drinking water too fast, got bullied for not having a dad and having no emotions other than being obsessed with rocks and was eventually sent to become a tool for the city against his mother's will. Then had to miss the death of his mom (aka the only person who didn't see him as a burden) and lived thinking no one was gonna be on his side and he was just gonna be a tool for the rest of his life. He suffers more throughout the story, rather than in backstory.

There's also "breaking through the clouds 2: swallow the sea
where MC's life was basically just straight up torture and trauma before he joined the police investigation force and is still hunted down by drug lords
while ML witnessed his entire family die to drug lords and barely survived with his life, still trying to find the dude responsible for his parent's death.
And it deals with topics such as murder, disfigurement, drug addictions, trauma, violence and all that stuff
