That was a nice circle back to the beginning but somethings are really confusing
1. Why did it look like Jiwoo's dad bonk him on the head? 2. Doc Lim still didn't explain properly what would happen if Guwon died. Did he mean the hounds would be more strict? 3. Why do the townspeople think they're being treated like livestock?
There's also something that came to mind: Doc Lim is wasting medicine. He doesn't know what's causing the kid to be sick. Remember he's a vet, he wouldn't know the signs and symptoms of specific diseases so he's doing a trial and error of the meds. "Welp that didn't work, let's try the next meds"
