I believe the implication is that he has the ability to switch bodies as his esper power. The director mentioned how it's a face and body of a civilian that disappeared that the Doctor is using currently. At first I assumed it was some sort of mind control ability, but the doctor mentioned that "unless they wanted a guide to off themselves every single day, they were to follow his demands." Something along those lines I believe? Meaning there's an implication that he can only cause the death of one person a day or there's a limit to what he can control. So with these I'm guessing even if he was killed, he'd be able to switch to another host and kill off another guide. Just my guess tho! I'm not even sure if I'm right.

He's an sss class. And you're right that's why they have to thread carefully. Because only seme is on the same par with him and that's just based on rank alone. We don't know the extent of his intelligence and skills from the years he's spent alive. Meaning even if seme were to attack and kill him, regardless of if he succeeds, it's still enough time for the doctor to switch to a new host(if we're going with my theory)
wait can someone explain why they didn't just kill the doctor? i get that they don't know how many people he brainwashed and he threatened to kill them etc. but i thought he referred to it as "counsel killing", i kinda imagined that as gaslighting or manipulation with words to drive then to suicide. so which wouldn't be possible if he was dead like i remember he talked about that one doctors boyfriend having to date an older woman and having a ton of workload and that he is probably super stressed. i assumed he'd be TALKING him into suicide cuz of the stress. am i missing something here? like do they die if he dies? how is he gonna manipulate them if he's 6ft under sjsjsj and dont bother replying if you say "reading comprehension is free" cuz thats just rude and im just asking
