it seem like the doctor is alpha female and she was in love with omega female when she was in school but that omega was not her fated pair so when her fated pair appear she left the other omega
I feel bad for the part where the staff talks to kaoru about how fated pairs can get complicated. I think the woman was either the one left behind or was the one who went for someone else. That really sucks
