There are people who have both male and female genetalia though? They’re not trans they’re intersex. Although the fact that in itself isn’t a plotpoint does seem like intersex fetishization, so the author is still a bit weird for that.
I genuinely still dont get why MC had lady bits, the only reason I can think of is that it's the new type of fetish material that has been appearing everywhere in yaoi now. If there not transitioning or it has no plot relevance and you want your ML to screw the lady bits make MC a girl then at that point, it just makes no sense why they can't have normal male anatomy anymore.
