I believe that's his mom and he was lead to believe that's his sister? Or he doesn't really see her as a mom and calls her sister idk man
so what happened is that rachel had her eggs taken from her since the dad was getting desperate for holy blood. so its not either or, rachel is both keith’s mom and keith’s sister since he shares the same DNA as the dad and her.
