The blood spot is thought to be proof that the marriage was consummated/ the woman was no longer a virgin. People used to think women bled during the first time. That’s not always true. He didn’t hurt her, it’s just a thing that they used to use as proof so it was included here.
Women dont need to bleed for their first time. It can happen but it usually means that it was too rough or not done with true care. I get that it can be used for proof but that plot point was only brought up in the second life. In her first life where he was portrayed as cold and indifferent i doubt he acted with care like in the second. Thats my issue

Hymens are very diferent in shape and nature, some so flexible they can survive multiple birth (!), some rigid enough to break from physical activities like horse riding… the pain or blood thing are also depending on that very strange vestigial piece of flesh.

It does say in her memories that he was gentle that first time. Plus since he apparently spent his time since late childhood on the battlefield they would likely both be virgins. Especially with the danger of assassination.
