I'm just gonna start this by saying that in every story it's different but in this one, they basically have a link for their emotions. Like what the alpha feels the omega feels, and vice versa. They might not have the same emotions at the moment per say but they "feel" for each other in a spiritual, physical, mental, and emotional sense.
Omega is getting a nosebleed bc apart from his preganancy, which is already taxing for him. He's feeling alpha's stress and it's adding even more to his mental and physical strain.
