Real, but I can understand why he didn't tell her. If your gonna go through hell, you don't want to bring your loved ones through it too. He didn't tell her because he believed it would be better than her trying to care of him and see him go through that.
Even if his reasons are valid. I don't think she deserved that, to die not knowing, to believe she wasn't loved is too sad idk I may be projecting here ┑(￣Д ￣)┍
