cauliflower ears are from repeated blunt trauma to the ear. This trauma creates a hematoma (a collection of blood) between the ear's outer skin and the cartilage, which can separate them. If the hematoma isn't drained, the lack of blood supply causes the cartilage to die and become replaced by fibrous scar tissue, leading to a permanent, deformed shape resembling a cauliflower.
