When Juheon left, Dohu kept everything exactly as it was, even his hair. Because cutting it would mean time has passed and that Dohu moved on. And he never did.
His long hair symbolizes the four years of waiting, of hoping the person he loved would come back and still recognize him.
He changed in every painful way, like wearing hats, hiding himself, and didn't keep his hair short because it was the one that reminded him of Juheon and cutting it would mean thathe accepted that their relationship is in the past, but its not .
