What i got is that he has a sickness that makes his lifetime expectancy to be short. So he's like, always trying to be ready to die (But it's still not sure if he would die even after being told that he only has 3 years left since that's apparently what happened before). Therefore leading to the conversation he had with his lover.

That is also what I understood:
He has a not yet curable sickness that's making the doctor tell him that he will have short life, but every time he exceeds their diagnosis
I'm abit confused? Does this mean his going to die soon?
