Two insane guys have a psycho relationship of abuse and madness, and one hallucinates when he's not enabling his captor-lover, while the other likes torturing and killing. Oh, and the pedophile uncles dead and gone but crazy little dude thinks he might still get up and do stuff to him. And there's a cop going nuts over this case, too, because we needed one more person pushed over the edge to make the psychological weirdness complete.
What's to not understand? :)
