I think it's PTSD, didn't he say the memories of his mom had been repressed? His mom raped him and being in such a similar situation with Bum made him remember, hence the hallucinations. He was having a PTSD flashback.

Is anyone thinking that Sangwoo has finally gone complete batshit and is hallucinating sometimes? He was hallucinating before he called Bum "mom." I don't know, it just doesnt make any sense. Especially when Bum supposedly drove away in the car with the girl but then he came out of nowhere from BEHIND Sangwoo directly after Sangwoo saw them drive away.
