The Doc raised an interesting point. Alex can't handle rejection. It seems obvious knowing his backstory, but it's something neither Clark or the Doctor took in consideration.
Neither did I, btw. I though Alex would kill the Doctor because it fit his M.O.: Doc was in love with him and an easy target. Or out of jealousy. Or both. Not like this.
The Doctor knew that Clark is unable to love and that he is the only one that came close. After all, he has always been the only human being Clark has made an effort to keep close. And he could see that Alex was obsessed with Clark.
Clark though he would be the next victim, but he forgot that the target needs to tell Alex they love him first. He never did. He made the guy go crazy about him and then rejected him.
He was raised by his wealthy alcoholic father. Although the staff treated him well, none of them stayed for long (because of his father). So he spent his entire childhood having to let go of people he grew attached to. Therefore he gets aggressive when people who love him try to leave.
