Nah i think he isn't trangender but cosplay as a girl because he is gay and want his crush (aka bestfriends) to love him. He is true to himself probably because he can be the center of attention of the mc.

....does it seem like the male friend had been transgender all along. I mean s(he) says s(he) feels like their real self for the first time in their life. If that's true then they were probably experiencing gender dysphoria. Am I looking too much into it? Anyway, I am going to consider the childhood friend as a girl from now on.
