The metaphor was explained in the bottom left corner of the panel by the author or team, its like the pain of having a wisdom tooth is as painful as ones first love, bcuz im assuming its referencing that first loves never work out?
I didnt get the wisdom tooth metaphor so i just assumed sooha was having dental health issues
