I don't think he yanked it with a purpose of hurting her. I mean maybe he was shock with the scars that he unintentionally pulled her hard. And when they apologize I think they're apologizing that he saw something that shouldn't be seen and the fact he "it's not your fault" probably means that it was they're fault that they had scars to begin with

It’s fine! Don’t worry! In my opinion they apologised because they thought about how the mother treated them, that’s why the girl had a small flashback about the brother apologising to the mum. I believed they felt as if they would get punished or something.
