Everything is explained on this page from the translator http://www.mangago.zone/read-manga/levelling_up_by_only_eating/uu/mpn_chapter-2302950/pg-43/

Yea, it’s understandable. Though most of the health/weight issues are with the story. There’s a lot of things that just do not make sense, especially since the MC has a supportive father and team, I’m guessing, of the best doctors and they’re attentive and care for him, so It seems iffy he still has such an issue. Now in the story it does mention he has a lot of other diseases, but then what about his care team that are supposingly always there for him? The only significant trauma that the MC probably has is something to do with his Mom that might of lead to this, but then once again how has the care team haven’t noticed it, do none of them understand psychology or mental health issues? Now One of the care team members might be secretly sabotaging him, but seems unlikely with this type of story. Anyway, I’m going to push back all these uncertainties and just enjoy the leveling up with cooking aspect and hope maybe they’ll answer some of these questions later on in the comic. ╮( ̄▽ ̄)╭
“Bulimia : a type of RARE disease...”
Ouaw that’s inaccurate, bulimia is not rare at all, why writing a fake definition ?
