Probably trauma. The "thing" that happened to him in middle school sounds to me like it wasn't sth good, there is also that scar on his head. I had a situation in high school that gave me a scalp scar along with being extremely repulsed when humans touch me and I also isolated myself from friends. I hope I am just jumping to conclusions though. It becomes really uncomfortable to read when one of the characters' trauma and their response to the trauma is similar to my own. I end up having to drop it. Then I feel bad for having to miss out on the work.
