I think the one i know is similar but the only difference is that the guy waited for like 20+ years and to find out he is the father to the guy who teleported to the past. I forgot the title.

I remember a story of a guy who travels to the future and falls in love with another guy there. He either gets into an accident or gets hit by something. I believe it was 5 years in the future. He meets a guy in the future who is 5 years older than him, and then he gets transported back to his normal time and waits five years to meet this guy again.
