nakyum‘s sister from the kisaeng house came and tried to talk to nakyum. seungho thinks that heena (the sister) was nakyum‘s accomplice in his escape so he forces him to stay inside until the sister goes away, saying almost the same thing to nakyum as the assassin also said. i guess nakyum was sick so he remembered the moment when he got released by jiwha and the assassin and kinda goes into shock ig? so once seungho comes back, nakyum begs for his life just as he did before, probably cause the fever caused him to think that he was back there where he got freed. seungho is confused and the other guy (idk his name tbh) came and saw how the whole thing unfolded (jiwha ordering an assassin to kill nakyum but chickening out before killing him) he then hints at seungho that it was jiwha who kidnapped but also returned him, which surprised and confused seungho cause he was convinced that nakyum tried to escape. so he kind of understood what happened. did this help u?
