Im thinking.... maybe because no one knows from his family and friends about his life in the village? Maybe he doesnt told anyone about his life and the girl he left because he doesnt want to hurt the her. Maybe he didnt get the chance to tell them because after the surgery, he immediately went into 2 years coma. After the coma, he went to a long recovery and didnt even remember her until a year ago when he saw the kdrama.
