I think it actually makes sense that she doesn't recognize him. From what I understood, Kim Hyeon's character, face, voice (he used a voice changer), and scent are all fake. Lee Wooshin's body is practically the only thing that could give her a clue, however, it was also mentioned that Wooshin bulked up after his mission, implying that he must've been thinner when he was acting as Kim Hyeon, so she still may not recognize him with that.
I don't know much about recognizing people with hands tho, I'm not really sure if it changes drastically when losing/gaining some weight. I think it would still be hard because people similar built may have similar feel to their hands. She was also blind so I think it's probably hard to compare sizes. But yeah, the curved dick is just a plot device to add some tension in this manhwa and I do think that even without it, this manhwa will still probably be an interesting read.
tbh, if I were in her shoes, I would also be like her. Cause searching someone who's precious to you, but you don't even know what he looks like because you're blind. That is the most tough thing to do, even if someone describe his features, you would still mistaken someone else as him since he was disguising himself with a silicone mask

About the hands point I can't remember if the study was about people who were born blind or experienced temporary blindness BUT, when asking them about what their favorite shape (?) was and showing them that shape, they couldn't recognize it even though they had felt it with their own hands. So this could explain how MC couldn't recognize ML, tho he did bulk up as you said
